First case Algernon Forbes,
male,white,58.Dr Rufus

The patient was referred to me February 10
with a suspected perforated ulcer

He had severe upper abdominal
pain and was in shock

Blood pressure was 70/40,he was ashen
grey and was in a cold sweat

The abdomen was rigid and
it was rebound tenderness

I ordered a transfusion to combat
shock and also morphine

No chest field X ray,Dr Rufus

It was my opinion that the patient
was too sick to go to X-ray

I agreed with the referring doctor's
diagnosis,obviously perforated ulcer

No doubts at all,Doctor

One always has doubts,Dr Pearson,

but I decided that all the symptoms
justified exploratory surgery

However,there was no perforated ulcer

I called Dr Jones for consultation but
before he arrived the patient died

Dr Pearson.Now is the time
for the autopsy findings.

No perforated ulcer,abdomen
entirely normal.

What was causing the severe pleuritic
pain was early development of pneumonia

Which could have been
discovered by chest X ray

It is very easy to have hindsight,
Dr Pearson loses no chance to remind us

I don't think Dr Pearson intended o..

Any man can make a mistake in medicine

but having been made that mistake
should serve as a warning

to others who can't see beyond the
area of their own surgical specialty
