from inmates Gerry Whyte and Sal Meti.

They alleged acts of brutality, extortion and harassment are taking place within the jail.

Those responsible include both prisoners and officers.

During our visit, a riot took place.

DI Warren managed to escape.

I was held hostage,

along with the dead officer, Reeves, and Gerry Whyte.

First question is - how could that happen?

Second question - Tierney's murder was brutal in the extreme.

Who could kill so sadistically and why?

Who knew Tierney had a pacemaker?

It belongs to a Dave Fenshaw. He's a porter. He was at home at the alleged time of Tierney's abduction,

but it's possible the killer borrowed his jacket.

Tierney was serving time - two counts for murder, one for armed robbery and abduction.

Was the killer someone Tierney and/or Symons crossed in the jail

and is now on the outside?

It could be an inmate, who's got contacts on the outside who's still banged up in prison.

They took advantage of the move here and the subsequent chaos.

DUNCAN: Obvious candidate - fellow prisoner Sal Meti.

He threatened Pace Tierney as he was being taken for surgery.

Every inmate was allowed a phone call when they were first admitted. Find out who Sal Meti spoke to.

Whoever killed these men, the connection appears to be the prison.

Check the records of every prison officer...

including Governor Houseman. Do not think

that because Tierney and Symons were hardened criminals,

that their deaths do not warrant fair and thorough investigation.

Initial tests suggest he ingested tetrodotoxin.

It's a heat-stable poison that blocks conduction along nerves.

His muscles - including those needed for breathing - would have become progressively more paralysed.

His heart rate and BP would have plummeted, his pupils would have become fixed. He appeared dead.
